Accreditation Requirements

For Initial / Reaccreditation of Physicians / Dentists

  1. Application Form (properly and completely filled up and notarized)
  2. Proof of remittance of Philhealth contributions (Validated MI-5 / RF-1)
  3. ID picture (2 pcs., 1"x1", name printed at the back)
  4. Accreditation Fee (P1,000)
  5. PRC ID (updated, photocopy)
  6. PMA / PDA card (updated, photocopy) or certificate of good standing from PMA/PDA or its local component society
    • if life member, certificate of good standing from PMA/PDA or its local component society is still necessary
  7. Residency Training Certificate (if any, issued locally or abroad)
  8. TIN card (photocopy)

Note: For withholding tax purposes (not a pre-requisite for application of accreditation)

  1. Certificate of registration - submitted only once unless there are changes.
  2. Affidavit - Sworn Declaration of current year's gross income - stamped received by BIR and submitted every 30th of June to 22nd of July of the CURRENT year to any PhilHealth office.

Important: Applications not completely filled up &/or lacking in requirement/s shall be returned without any action.

For Renewal of Physicians / Dentists

  1. Application Form (properly and completely filled up and notarized)

    • - if life member, certificate of good standing from PMA/PDA of its local component society is still necessary

Note: For withholding tax purposes (not a pre-requisite for application of accreditation)

    If a new TIN is issued by BIR, a photocopy of new TIN card should also be submitted to avoid problems in claims processing

  4. Accreditation Fee (P1,500)
  5. PRC ID (updated, photocopy)
  6. PMA / PDA card (updated, photocopy) or certificate of good standing from PMA/PDA or its local component society
    • if life member, certificate of good standing from PMA/PDA or its local component society is still necessary
  7. Specialty Board Certificate as Diplomate / Fellow issued by Philippine Specialty Board
  8. Certificate of Good Standing as Diplomate/Fellow from the Philippine Specialty Board (must be submitted if specialty board certified for more than 2 years)
  9. TIN card (photocopy)

    • if life member, certificate of good standing from PMA/PDA or its local component society is still necessary
  7. Certificate of Good Standing from the Specialty Society as Diplomate/Fellow from the Philippine Specialty Board (must be submitted if specialty board certified for more than 2 years)
