Accreditation Requirements
For Initial / Reaccreditation of Physicians / Dentists
- Application Form (properly and completely filled up and notarized)
- Proof of remittance of Philhealth contributions (Validated MI-5
/ RF-1)
- ID picture (2 pcs., 1"x1", name printed at the back)
- Accreditation Fee (P1,000)
- PRC ID (updated, photocopy)
- PMA / PDA card (updated, photocopy) or certificate of good standing
from PMA/PDA or its local component society
- if life member, certificate of good standing from PMA/PDA or its
local component society is still necessary
- Residency Training Certificate (if any, issued locally or abroad)
- TIN card (photocopy)
Note: For withholding tax purposes (not a pre-requisite
for application of accreditation)
- Certificate of registration - submitted only once unless there are
changes.
- Affidavit - Sworn Declaration of current year's gross income - stamped
received by BIR and submitted every 30th of June to 22nd of July of
the CURRENT year to any PhilHealth office.
Important: Applications not completely filled up &/or
lacking in requirement/s shall be returned without any action.
For Renewal of Physicians / Dentists

- Specialty Board Certificate as Diplomate / Fellow issued by Philippine
Specialty Board
- Certificate of Good Standing as Diplomate/Fellow from the Philippine
Specialty Board (must be submitted if specialty board certified for
more than 2 years)
- TIN card (photocopy)

from the Philippine Specialty Board (must be submitted if specialty
board certified for more than 2 years)
