PhilHealth InfoDesk In Umingan, Pangasinan - August 16, 2010

TO bring its services closer to its members, the Philippine Health Insurance Corporation (PhilHealth) recently established an Information and Assistance Desk in the Municipality of Umingan, Pangasinan.

Located at the ground floor of the Umingan Municipal Hall, the infodesk is open every second Wednesday and last Friday of the month, from 8:00 AM until 5:00 PM.

Felimon V. Petinez of PhilHealth Service Office Urdaneta said that the assigned personnel at the desk will be accepting premium contributions, and requests for Member Data Records (MDR) and PhilHealth Identification Number (PIN) cards. Accreditation and collection reports, membership applications and benefit claim documents will also be accepted.

Forms for benefit claims as well as membership registration are also available at the desk, while various information materials can also be obtained from the desk.

With the establishment of the desk, PhilHealth members from Umingan and nearby towns will no longer travel far to gain access to PhilHealth’s services. Umingan, Pangasinan is located at the easternmost part of Pangasinan and is bordered by the towns of Balungao and San Quintin, Pangasinan and Lupao, Nueva Ecija. It has 58 barangays, has a population of 62,497, and is more than 50 kilometers away from the nearest PhilHealth Service Office, which is the SO-Urdaneta. (Sibelle Estrada, PhRO I)
